ODP recently issued a "Wage and ERE Survey" requesting providers to submit the forms by November 1st.   According to ODP, this is not a steadfast deadline, and you can submit the survey at any time.  The submission of the forms is voluntary, unless you are submitting a Needs Exception request, in which case the survey is mandatory.
