On April 29, 2011 ODP issued revised forms and insturctions for ICF/MR Budget Modification requests. Budet Modification Requests, which are applicable to programs who have received approved waiver requests, must be submitted by June 30th of each fiscal year. The primary changes to these forms are:

  • Changes "OMR" references to "ODP"
  • Added new " Justification" form, which was previously provider's own format

AZTAC has reviewed these forms. Note that there is a formula error in GA Personnel Costs, as well as minor formatting issues. Contact us for more information if you are planning to submit a budget modification request and need assistance.
